If you don't submit the Statement of Use or a request to extend the deadline, you'll lose your trademark and all previously paid fees.

After the USPTO issues the Notice of Allowance, you have six months to submit the Statement or ask for a six-month extension. You can ask for an extension a total of five times. You have to request the extension during the period, one extension at a time. If you fail to submit the Statement of Use within 36 months after the NoA was issued, your trademark will be abandoned.
